Job Description

Are you fed up of being confined on a ward Would you like to know what happens to your patients when they leave hospital Would you like to break out, get some fresh air and see PATIENTS in their OWN HOMES to CONTINUE their CAREWe are seeking kind, caring and dedicated registered Nurses to compliment the Havering Community Health and Social Care Service team. Our Community and District Nurses support the delivery of 24hr holistic health care to support our strategic vision of the ‘Best care by the best people’As a member of our community multi-disciplinary team, you will work within a primary care network team alongside District Nurses, Specialist Nurses, Therapists and Health Care Assistant’s to meet the needs of complex community delivered patient care supporting early discharge and avoiding admissions to the acute sector.This includes;

  • Intravenous medication administration,
  • End of life care,
  • Managing the deteriorating patient,
  • Complex and simple wound care including VAC therapy and compression therapy,
  • Venepunture,
  • Utilising advanced physical assessment skills and many more clinical nursing skills.
  • We will be recruiting for the North, South and Central localities..If this post is for you, you will demonstrate an awareness of current evidence based approaches to patient care and be actively involved in the development of local strategies and contribute to service improvement and redesign.
