We are looking for an enthusiastic and motivated Practice Nurse to join our supportive and friendly team, working up to 37 hours a week.The successful candidate will undertake a range of nursing assessments and provide appropriate care working independently as appropriate in line with practice policies and protocols.The practice nurse will be involved in identifying practice population needs, general practice nurse duties including Chronic Disease management in areas of diabetes, asthma and COPD, also working to GMS contract, Quality and Outcomes Framework, along with National and local enhanced services is a vital element of the post.Additionally the successful candidate will be encouraged and supported to undertake any further training required to ensure appropriate role development.As a training practice we support and mentor student nurses and encourage personal development
