Job Description

Care ManagerBlaenavon£31,050We are working in partnership with a long standing client, who are currently seeking a Practice Lead for their Blaenavon service. They provide support to adults with Autism and complex care needs.Duties of the role include (but not limited to)

  • To carry out behaviour analysis, in conjunction with the Complex Behaviour & Autism Specialist, compile reports, ongoing monitoring and evaluation
  • To ensure there are plans in place that promote peoples communication and support them
  • with making choices and decisions.
  • To ensure that support is person centred and tailored to needs, goals and agreed outcomes
  • of each person supported.
  • To lead and develop practice within services based on organisational policy, best practice,
  • contractual and regulatory requirements and legislative frameworks.
  • To lead Complex Behaviour and Autism services within your area of responsibility to deliver
  • creative, person centred, outcome focused support to each person supported.
  • To take appropriate and immediate action following procedures where you or others observe
  • signs and symptoms of danger, harm and abuse or where this has been disclosed.
  • To formulate rotas and manage staff resources effectively, in line with individual need and
  • budget requirements
Skills and experience required for the role:
  • QCF level 4/5 management qualification (or equivalent)
  • 2 years practical experience of supporting people with autism and/or complex behaviour
  • 2 years experience within a management or supervisory role
  • Ability to understand, analyse and monitor a range of complex behaviours
