The successful candidate will:

The Lifestyle Coordinator is responsible for developing and implementing a varied and engaging program of lifestyle opportunities and activities for the residents in our nursing home.An approach of making lifelong passions a reality and life long relationships meaningful is essentialThe goal is to provide residents with a wide range of opportunities to continue to socialise, learn, and enjoy themselves.Essential Duties and Responsibilities:
  • Assess the interests and abilities of each resident
  • Develop and implement individualised activity plans in our care planning software , this will include capturing a wide range of information about the individuals Lifestyle choices, passions and preferences.
  • Prioritise family connections and friendships whether in the home or in the community
  • Organise, plan and lead a wide range of group activities
  • Work with other staff members to ensure that residents are able to participate in our lifestyle events and activities
  • Evaluate the effectiveness of the lifestyle events and activities and make adjustments as needed
  • Collaborate with other departments, to ensure meaningful outcomes are in place , e.g: the catering team for the mealtime experience
  • Maintain records on our care planning system of resident participation and progress
  • Stay up-to-date on the latest trends in the activity and lifestyle world including a knowledge of NAPA and my home life events
