Job Description

Homecare Nurse PlymouthScope:Fantastic opportunity for a high-calibre ICU/ITU, Critical Care, Surgical or Nutrition nurse to join a global leading manufacturer of healthcare products as a Homecare Nurse.Specialist training will be provided for the successful candidate in this highly progressive role. In addition to training, you’ll be provided with everything you need to succeed, including a company car, fuel/expenses card, laptop and mobile.Responsibilities:

  • Provide individualised quality patient care to all homecare patients in their home setting underpinned by current local and national guidelines and policies.
  • Demonstrate a flexible approach to support the nurse led service -working in accordance with the nursing rota, on-call rota and the Annual Leave Policy for homecare Nurses.
  • Work with the Senior homecare Nurse and Regional Lead Nurse in discharge management, following relevant processes, to provide a seamless service.
  • Support the Installation of new homecare patients - complete all relevant documentation and escalate any issues.
  • Administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
  • Provide training, education and support to patients/carers with regards to the equipment and infusion devices required for infusion/administration of therapies to independence.
  • Provide training, education and support to patients/carers in the connection and disconnection of PN and IV therapies to independence.
  • Provide training, education and support patients/carers in the care and management in accordance with local policies and national guidelines to independence.
  • Report any clinical changes in patients to the relevant Senior homecare Nurse or Regional Lead Nurse or Trust.
  • Always maintain contemporaneous nursing care records in accordance with the record keeping guidelines.
  • About You:
  • NMC registration
  • Intensive Care / High dependency / General surgery/ nursing care experience at a level equal to top of Band 5
  • Current clinical experience minimum of 2 years
  • Competent in the management and delivery of PN and other IV Therapies
  • Competent in the care and management of CVADs
  • Blood samples from CVADs
  • Cannulation/Venepuncture experience is highly desired
  • Understanding of national standards and guidelines for IV infusions/CVADs
  • Excellent Communication skills
  • Basic IT skills
  • Good time management
