NHS Cambridgeshire & Peterborough CCG is recruiting a number of resourceful, experienced and confident clinicians to the post of Continuing Healthcare Nurse.As a Continuing HealthCare Nurse, you will work as part of the Complex Cases Team based in either Peterborough or Cambridge. We have an additional Team also based in Huntingdon.The post holder will:Deliver effective assessments for patients who may be eligible for NHS funding ensuring their needs are identified and case managed in order to provide the appropriate care.Regularly attend visits to patients’ homes to ensure the care is effective and the patients’ needs have not changed.Carry out annual reviews as per the NHS Continuing Healthcare Framework Guidance.Complete a ‘duty’ role assisting the Case Management Team with daily enquiries, this will also include liaison with the Acute hospitals to assist with discharge of patients from hospital.Adaptive; CHC is a fast paced, forever changing and responsive area of work. You will need to be IT literate and confident on the telephone as well as face to face. You will need to use your time effectively and have the ability to prioritise.Conscientious: You will want to provide the best assessment of a patients needs and have an eye for detail.You will complete all aspects of NHS Continuing Healthcare and strive to improve processes and services. You will respond and report safeguarding concerns working closely with system partners.You will provide expert clinical knowledge and advice on NHS Continuing Healthcare to the wider health and social network.As the successful candidate you will:Possess a current nursing qualification RN, RNMH, RNLD all welcome to apply.Have some experience of CHC – Training will be givenWork flexibly to support both the senior management team and clinical teams. Ensure that learning and good practice is shared across the team to standardise processes and achieve service improvement.Adapt to new ways of working within an evolving environmentTravel is a key part to this role. You must be able to drive and willing to travel across the CCG areaBe solutions focused.The CCG is currently working through a period of change regarding Ways of Working and on appointment into this role the successful candidate will be required to work mainly from your home with a from time to time visits to our new office location planned in Ely, Cambs. The ability to attend locations throughout the Cambridgeshire and Peterborough geography will be essential for this post
