Job Description

Contract Type: PermanentLocation: Community BasedSalary: £20,872- £22,143 pro rata per annum (depending on skills and experience)Working Hours: 28 per weekClosing Date: 27 June 2022Interview Date: To be confirmed (interviews will be conducted through Zoom or Teams)We are looking to welcome an approachable, enthusiastic, and knowledgeable, community-based Dementia Adviser to join our supportive and compassionate team, focusing on supporting people affected by dementia across West Lancashire. This is a community-basedrole with some homeworking elements.There is no direct care element to our role. We provide advice, support and guidance to people affected by dementia, passionately promoting their independence, improving sense of well-being, putting people in more control of their lives.This is a great opportunity to join our passionate and empathetic team providing personalised advice and support to people diagnosed with dementia and their carers and to promote our service and build relationships with a range of health and social care professionalsin the voluntary and private sector.What you will do;

  • Offering a vital, compassionate advice service to support and guide those affected by dementia.
  • Provide community based, face to face services for an increased level of support for more complex cases.
  • Managing referrals, assisting clients in assessing their information and support needs in a person-centred manner.
  • Problem solvinghelping people find ways to better live around symptoms and challenges that come from their dementia.
  • Providing knowledgeable and trusted information regarding services and benefits available and, where appropriate, assist clients in accessing them.
  • Facilitating group services for people with dementia and for carers
  • Signposting clients to a choice of suitable other sources of help, where appropriate, and networking local services to strengthen a Dementia Friendly Community
  • Developing a proactive approach in reaching people with dementia and carers who may not otherwise access our services.
