Specialist Assessment Co-ordinator: Community Mental Health Team

Location: OldhamBand 6Hours: 37.5 hours per week, Monday-FridayMain Duties & Responsibilities:

The role is to work as assessment and/or care coordination worker for referrals received into the team from other teams, i.e. HTT, Inpatient Wards and Access.

To act as a secondary care assessment worker/short term intevention, carrying out assessments of clients referred to CMHT as they are suffering with a severe and enduring mental illness requiring secondary care. To support and supervise junior staff withinthe team. To at times manage and supervise the team.
To support the assessment of the needs of service users by utilising specialist research validated assessment tools
To support the risk assessment process through development of and application of skills in identification of risk, implementation of and evaluation of risk management plans.
You will be expected to provide a full holistic assessment and risk assessment, with the aim to establish if the person referred requires longer term support from the CMHT which would result in transfer to care co-ordination, or alternatively to identifyshort term intervention which will be provided within the assessment function of the team.
