Job Description

Job IntroductionThis position is an exciting opportunity to improve the pathway for alcohol and drug users from the hospital to the community. The post will involve close partnership working with Royal Oldham Hospital, in particular the Alcohol Liaison Team, to assess andscreen service users, deliver advice and information and hold a case load of specific clients as they are discharged from the hospital. You will join a committed and friendly team at Turning Point Oldham who take a multi-disciplinary approach with supportfrom clinical and psycho social interventions staff, peer mentors and a strong network of partner agencies .As a Advanced Recovery Practitioner we offer a starting salary of £29,528 with annual pay progression up to £31,937 (You may be offered a higher starting salary. This is dependent on experience)You will have opportunities to progress your career with a structured learning journey and clearly defined career pathways that will enable you to achieve your long-term goals and work to your strengths.Role ResponsibilityTo assist in delivering Turning Point’s Substance Use strategy by planning and providing high quality, innovative care which reflects our person-centred values and the high levels of ambition we have for the recovery of the individuals for whom we providesupport.Key Responsibilities:

  • To deliver Hospital in-reach to A&E departments, medical wards and GI wards.
  • Contribute to the development of after care packages in conjunction with the Hospital Alcohol and Drug Liaison Nurses prior to hospital discharge.
  • To contribute to the development of seamless care pathways for continuation of detoxification regimes where these are initiated in hospital and are appropriate to continue in the community.
  • To complete assessments, provide early engagement, advice and brief interventions for those identified as having substance use support needs by Hospital Drug and Alcohol liason nurses and develop packages of care within the community
  • To work with families / significant others of those identified as having drug and alcohol support needs upon release from hospital to maximise social recovery capital and to support families / significant others. To involve them in recovery plans wherepossible.
  • Work flexibly in a variety of settings to create care and support plans that meet the needs of the service and clients in accordance with the recovery agenda
The Ideal CandidateYou’ll have an understanding of substance and alcohol use and the challenges and opportunities for individuals on a recovery journey. However, we also recognise that many people have transferrable skills from other backgrounds such as mental health, housingor other health and social care provision. The roles are varied, so flexibility, an ability to work dynamically on a one to one and group basis and knowledge of relapse and other barriers to recovery will be vital but we can help to shape these skills whereneeded.
