Job Description

A not-for-profit organisation who provides services on behalf of the NHS and local authorities are looking to recruit a Patient Coordinator to work in their office on the outskirts of Newcastle. This is a 6-month fixed term contract, working a 37.5 hourweek Monday to Friday.A pivotal role within the organisation, you will be the first point of contact for patients being referred into the service, and act as a liaison between the patient and clinical team. As a Patient Coordinator, you will ensure the patients experience ismanaged efficiently and seamless throughout.Duties Include:

  • First point of contact to respond to referrals or calls from patients
  • Follow risk protocols and procedures and liaise with the duty team to make sure any relevant risk is communicated to the clinical team
  • Update the CRM with patient information
  • Arrange/ cancel patient appointments
  • Attend and contribute to staff and role meetings, forums
  • Keep records of clinical activity
About you:
  • Worked within a high-volume environment, preferably within mental health
  • Self-motivateded and able to manage own caseload
  • Able to work under pressure and work within the boundaries of confidentiality
  • Experience working with the public, either within a healthcare setting or call centre
  • Good IT skills
  • Preferably experience in nursing, social work, occupational therapy or counselling
