Job Description

**Please note this position is for current East Cheshire NHS Trust employee’s only**East Cheshire Trust is looking to appoint 2 community coaches who will work as change agents to transform and develop community services in 2 of our 5 community care teams. The successful candidate will lead the delivery of transformation across these 2 care communities in order to deliver high quality care for our local population.At East Cheshire, we are committed to transforming local services and are working with partner organisations to deliver our strategy and develop a model for place based systems of care to improve service delivery for both patients and staff. In return, we are looking for someone who is committed to ensuring that all services within the locality deliver community services are delivered in an effective, efficient and safe manner.Alongside the Clinical Lead for each care community, you will be the driving force to inspire your teams to take up the challenge of introducing new ways of working, always focused on our patients and the population as a whole. You will provide strong and effective leadership and line management to the teams within your designated care communities, working alongside Community Coach colleagues to strengthen integration of services and locality working relationships.
