Our client is a local authority based near Hackney is looking for experienced Qualified Social Workers for their Hospital discharge team. The aim of the Integrated Discharge Team within Homerton University Hospital is to help patients to return safely to their own home or care home with the right level of care and support.The discharge planning team works with the multi-disciplinary team on the ward - including the patient, doctors, nurses, therapists, specialist nurses and social workers - to carry out each patients needs assessments and make the necessary arrangements for patients to be discharged safely and in a timely manner.This is an opportunity for you to join a service with positive reviews from its staff.
