Job Description

The post holder will play a key part in a multi-disciplinary team, whose aim is to help people with long-term health conditions improve their health management and return to work.As a Physical Health Practitioner you will utilise your specialist Physiotherapy skills of advice and guidance on management of common musculoskeletal, cardiopulmonary and neurological conditions, exercise prescription and return to work advice. The post holder will take a holistic approach to long-term health condition management and work closely with employment specialists (Keyworkers/Caseworkers), other Ingeus employees and health services to help people improve their lives.Direct Delivery to Participants

  • Conducting biopsychosocial health assessments to determine participants health needs; face to face / telephone / videoconferencing
  • Offering advice and guidance to participants with regard to their health condition and empowering individuals to manage their health more independently and effectively
  • Producing tailored health action plans for participants seen on an individual basis or with the employment specialist (3-way appointment), focusing on improving their health and moving them closer to work
  • Providing ongoing face-to-face and/or remote based health advice and guidance to individuals to help them manage their health condition for a small caseload of participants that need more intensive 1:1 support
  • Delivering and developing educational group sessions such as Managing Low Back Pain, Living Well with Chronic Pain, Managing COPD and The Benefits of Exercise
  • Monitoring participants’ individual health and wellbeing throughout group activities and reporting significant changes which may impact positively or negatively on a participant’s recovery or return to work to the employment specialist
  • Providing return to work guidance and offer in-work support where required
  • Delivering physical based classes such as walking groups, relaxation classes, circuit training, Tai Chi and Pilates (training for these activities will be provided as appropriate to the needs of the business)
  • Screening participants for suitability for exercise classes
  • Ensuring participants are able to access appropriate and up to date condition specific self- help material
  • Referring participants to specialist services, agencies and organisations where appropriate
  • Maintaining accurate records
  • Ensuring that all participant activities are recorded accurately and appropriately on our systems in a timely manner, as per KPIs / standards
  • Updating participant health action plans after appointment and group sessions
  • Recording all other supporting documentation as appropriate onto participants’ files
  • Providing monthly statistics in relation to activity levels at the end of each month
  • External Stakeholder Relationship Management
  • Working with Senior Health Practitioners and the wider health and wellbeing team to build relationships with external organisations such as the NHS and other specialist providers of health and wellbeing related services;
  • Building referral routes into lifestyle services such as physical activity schemes
  • Internal Stakeholder Relationship Management
  • Demonstrating excellent communication with the Health and Wellbeing team and all other operational staff;
  • Generating referrals into the services you run; marketing to teams to maximise uptake
  • Providing in-work support, which may involve liaising with employers
  • Providing upskilling or training to operational staff to continually improve the service offer to participants
  • Manage Risk Related Issues Appropriately
  • Escalate any risk issues as per Ingeus Safeguarding Policy.
  • Ensure your Senior Health Practitioner or Line Manager is aware of any risk issues which have been escalated.
  • Continued Development
  • Participating in monthly review meetings to discuss your performance in the role and delivery of the Health and Wellbeing service;
  • Engaging in supervision with a Senior Health Practitioner as required
  • Participating in an annual professional development review to identify areas for ongoing personal and professional development;
  • Participating in training and development opportunities provided;
  • Taking ownership for your CPD and maintaining your professional registration.
