Job Description

About the Service / DepartmentGrove Outreach service enables Royal Borough of Kensington & Chelsea residents to avoid, and in particular to reduce the need for, prolonged hospital admissions and long-term dependency on mental health services. We promote recovery and prevent social exclusionamong service users with mental health support needs.This service deliver community based, person-centred and holistic support to service users who are aged over 18, who have a severe and/or enduring mental health needs and living in the Royal Borough ofKensington and Chelsea.The overall aim of support provision is preventing hospital admissions and facilitating earlier discharge, promoting recovery, social inclusion and helping service users to maximise their independence and wellbeing, access to employment, training, and othermainstream activities.The service operates Monday to Friday 09.00 – 17.30, excluding bank holidays.Key Responsibilities:

  • Responsible for the care and well-being of service usersusing holistic, person-centered support planning with SMART targets and the recovery star
  • Visit service users within their homes or appropriate community locations
  • Devise effective interventions for service users who are ‘hard to engage
  • Assist with accessing medical services and the local community mainstream services.
  • Ensure that needs & risk assessment and SMART support plans are in place and reviewed at agreed frequencies
  • Formulate, implement and review support plans with service users and associated professional participation
  • Liaise effectively with other agencies where appropriate
  • Encourage on-going engagement with existing networks including faith-based groups and informal sources of support.
About You:
  • Experience of working with people requiring support related to mental ill health and complex needs.
  • Experience of delivering support to people within a community/outreach setting.
  • Ability to support service users to achieve their goals & aspirations.
  • Excellent knowledge of issues related to signs, symptoms, and treatments of mental illness; ability to assess service users declining mental health and take appropriate action.
  • Knowledge of health and safety relevant to the role including lone working within a community setting
  • Knowledge of key working, person centered support planning systems and the recovery approach
  • An understanding of equality and diversity and how it applies to the job role and the importance of promoting service users rights, privacy dignity and choice.
  • Knowledge of welfare benefits entitlements appropriate to the user group
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately.
