We are seeking an enthusiastic and innovative Consultant in Rheumatology & Acute Medicine at the Royal Free Hospital.The post holder will be active in rheumatology with a special interest in sarcoidosis. They will perform clinics in general rheumatology and in-patient ward rounds. They will establish a Hot Clinic service allowing rapid review of urgent referrals with suspectedrheumatological disease from ED. In addition the appointee will provide 50% of their job plan to the Department of Acute Medicine working 1 week in 12 on the Acute Medical Unit (AMU, ward 8N) with the other consultants in Acute Medicine, providing inpatientcare for those admitted via the acute admissions system, principally the Emergency Department (ED) and will contribute to on calls and SDEC clinics (same day emergency care clinics) for a proportional number of PAs to their 50 per cent acute medicine contributionjob plan. Out of hours and weekend cover is currently provided on a 1:12 basis. There are thus 2 consultants on duty each day for acute medicine.The Centre for Rheumatology has an international reputation for clinical rheumatology, management of connective tissue disorders, teaching and basic research. The unit has in-patient beds on 11 West ward, and a highly active and successful day care unitheld on PITU 2 North. The appointed consultant will be fully involved in the clinical and post graduate activities of the department leading the Postgraduate Rheumatology Seminar Programme.The Royal Free Hospital AMU is located on ward 8N and has a total of 32 beds. All patients admitted under the care of Acute Medicine or Health Services for Elderly Patients (HSEP), should initially be there in order to provide high quality acute medicaland multi-professional care, thereby allowing prompt and accurate diagnosis, appropriate treatment and timely discharge. There is close liaison with the Respiratory team, based on the adjacent ward, who provide rapid input for those patients presenting withacute respiratory problems and take over their care according to agreed parameters.Patients with other conditions requiring specialist input are referred as necessary and those with general medical problems requiring a longer inpatient stay are looked after by the General Medical team. Various agencies are available to provide Rapid supporteddischarge which is provided by the Post Acute Care Enablement (PACE) team. PACE provides immediate follow up on discharge from hospital, for instance for patients requiring anticoagulant monitoring, completion of intravenous antibiotic therapy, follow up bloodtests or those requiring extra support due to disability.The working day for both the AMU and medical take consultants begins with the morning post-take ward handover round or Morning Report at 08:00; a multidisciplinary board rounds follows at 09:15, on both the AMU and the Adult Assessment Unit (AAU, situatedadjacent to the Emergency Department, ED, on the lower ground floor) which includes relevant members of the nursing team, social workers, occupational and physiotherapists and bed managers. In-patient rounds take place thereafter. The AMU consultant works08:00-17:00; the medical take consultant continues post take rounds until evening handover at 20:00.In addition to their qualifications in acute medicine, the prospective post holder will have qualifications and experience in rheumatology to complement the speciality interests of other physicians directly involved in the delivery of acute phase care.
