Job descriptionCare CoordinatorSalary - £25,000 - £30,000 per annumHarmony Healthcare is looking to recruit an experienced Care Coordinator to join our friendly team.Applicants should have a good understanding of the Homecare sector and suitable coordinator experience, with skills in rostering, IT, administration, and customer service.This role requires Care Coordinators to be up to date with all relevant policies, procedures, and regulations.At Harmony Healthcare we specialise in the delivery of Nursing and Care services for children and adults living with complex physical disabilities and life limiting conditions. We deliver our Nurse led services in our clients own homes and workin partnerships with CCGs/NHS, Case Management companies, Local Authorities and Private Clients to improve patient outcomes and experience. Our focus is to lead with quality and put our clients at the heart of everything we do.Job SummaryTo be responsible for managing complex care clients and ensuring they are suitably staffed into the future with a core stable team of carers.To liaise with clients on a regular basis.To gain a clear understanding of the care required and clients health needs and clear guidance and instructions explained to any potential and existing carers.To discuss with clients their feedback and keeping them regularly informed of changes to the team and changes to the shifts.To keep clients safe by building resilient stable teams around each client that contain enough numbers of carers to accommodate sickness and other absences.To strive for excellent customer service alwaysTo act on the clients behalf internally if any issues ariseTo keep rotas staffed to at least 12 weeks in advance.To optimize utilization of carers wherever possibleTo proactively share and communicate shift plans with all relevant individuals and updating them in a timely manner of any changes madePromptly responding to and acting on shift cancellationsEnsuring uncovered shifts are addressed and managers and families informed of actions being taken in good time.Being actively involved in the on-call process on a rotational basis.To comply with all company policies and proceduresTo comply with all our regulatory requirementsTo maintain confidentiality always and in line with the companys confidentiality policy.To carry out the monitoring of the services to ensure that it meets the targets of the company, contractual requirements of the commissioners and escalating to the Operations Manager, or Clinical Team if necessaryTo work closely with the Recruitment Team, to ensure that additional carers are sourced in a timely mannerTo work closely with the on-boarding team to ensure that new clients are smoothly and successfully on-boardedTo report any issues concerning the service provided to their line managerTo work closely with the Clinical Team to ensure all clinical advice and plans are appropriately factored into the operational planTo lead on the individual care packages and teams providing service advice and support to clients, families, and care teamsReport to Clinical Team any concerns and changes to health needs so that action can be takenTo work as a team in trouble shooting and identifying cover for short notice cancellationsIf you believe you meet the criteria and are suitable for the position then please apply below.
