A Vacancy at NELFT NHS Foundation Trust.COMMUNITY RECOVERY TEAM- Redbridge West Adult Mental Health Services(5 sessions)This post is attached to Dr Abou-El-fadl, Consultant Psychiatrist. It is designed along the principles of the Recovery Model with emphasis on promoting reintegration and maintenance of recovery aiming at eventual discharge back to primary care.Dr Abou-El-Fadl covers the community recovery service of half the West Sector of Redbridge catchement area (CRT West). The area is socio economically mixed and culturally diverse. The CRT is based in Mellmead House in Wanstead and it is a well-established Multi Disciplinary Team with full integration with Social Services.The service is firmly linked with the Home Treatment Team and the inpatient service. The HTT is represented in the weekly zoning meetings to ensure full liaison and shared management perspective of relapsing or acutely ill patients. Admission to the inpatient is usually organised through HTT; and the ST4-6 would have a vital role in linking the CRT with the male inpatient to ensure continuity of care. They share the community perspective with the inpatient unit regarding the nature and history of the condition and contribute to the management plan trough attending ward rounds. The inpatient beds are in Sunflower Court at Goodmayes Hospital.Clinical ExperienceThe trainee will have the opportunity to gain experience in the management and follow up of patients in the community along the principles of the recovery model with the aim of facilitating reintegration in the community and ultimately discharge back to primary care. They will hold regular outpatient clinics with Key workers and families, conduct joint home visits with the team staff to patients’ homes and residential placements. The trainee is expected to become Section 12 Approved as soon as possible. They will gain experience in the use of the Mental Health Act by dealing –under supervision- with a number of CTOs as well as restricted section 37/41 cases; including the preparation of periodical reports and attending MHRT hearings on behalf of the RC. Dr Abou-El-fadl is an expert witness and will coach the trainee in writing medico-legal reports to the court and will assist the trainee in attending court whenever possible. The trainee will be liaising with HTT with regards to referring acutely ill or relapsing patients, contributing to the formulation of the management plan and maintaining contact until the patient is improved enough to be taken back by the team or be transferred to the inpatient service. When patients are admitted to the inpatient unit the trainee will be involved in liaising with the inpatient consultant and the ward team with regards to the nature of the condition, the management plan and preparing for discharge. The patient key worker is also involved in this process. This would be achieved through attendance of ward rounds with the Inpatient Consultant and maintaining contact with the inpatient staff and the community key worker. This is to ensure continuity of care through briefing the inpatient unit with perspective of the community service of the particular patient. The post provides extensive experience in the community through individual assessments re-appraisal of diagnostic evaluations and working towards recovery through joint work with the CRT members.TeachingThere are ample opportunities for teaching. Dr El Fadl teaches 4thYear Medical Students who attend throughout the year for 6-week periods for their psychiatric attachment. There are opportunities for teaching ST1-3 Trainees including exam focused training as well as occasional teaching for other disciplines.Training Supervision and CPDClinical supervision will be provided by Dr El-Fadl as appropriate. There would be educational supervision for 1 hour per week. The trainee is expected to attend the local teaching on Wednesday afternoon and is encouraged to take study leave for appropriate courses.ACUTE MALE INPATIENT UNIT- Ogura Ward, Sunflowers Court(2 Sessions)Ogura ward provides inpatient assessment and treatment of all mental disorders associated with the general adult population between the ages of 18 to 65 years. It has 20 beds. Ogura ward is a male only ward and provides services mainly to men from the London Borough of Redbridge. About 60% of patients are under section of the Mental Health Act. The average length of stay target is 21 days.Patients are admitted to the ward via the Home Treatment Team (HTT), who serves as gatekeepers to inpatient care and is also involved in the early discharge of patients from the ward. Patients come to HTT from the local GPs, local A&E departments, Access and Assessment team, Community Recovery Teams, Early Intervention in Psychosis team, and the police via the Section 136 suite.Ogura ward works closely with the HTT in order to move patients back to the community as soon as possible. A HTT handover meeting takes place every morning to discuss all the patients on the ward in order to identify patients who could be discharged early, or go on leave from the ward under supervision of the HTT. Ogura ward falls within the Inpatient and Acute Directorate (IPAD), which is mainly a Mental Health focused directorate and most of its services are based at Goodmayes Hospital. Ogura ward is 1 of 3 male acute wards in NELFT providing mental health services to the four Boroughs of Havering, Barking & Dagenham, Redbridge, and Waltham Forest. The 3 wards therefore work flexibly in conjunction with each other when beds for a specific catchment area are not available on the dedicated ward. The wards across IPAD support each other in times of staff shortage or when dealing with serious incidents.All patients on the ward are discussed and assessed weekly during ward review meetings. The medical team sees new admissions within 24 hours of admission. Patients also have regular input from the medical team and nursing team outside of the ward review meetings. Physical health monitoring is also high on the agenda, and all patients will have baseline physical health examinations and investigations.There are currently 1 full-time Specialty Doctor, 1 full-time GPVTS, 1 full-time FY1 doctor, 1 part-time ST (2 sessions on Tuesday) attached to the team.Principal DutiesThe postholder will support the Consultant Psychiatrist in providing clinical leadership for the General Adult Male Acute Inpatient Ward. The role is a senior one requiring the ability to function autonomously when appropriate.
