Job Description

The Assessment & Support Team will act as a gateway to internal support services and teams, and a central source of expertise on all areas of support. The team will take on a coordination role for the most complex support cases. As Team Leader, you willline manage a small team of Assessment & Support Specialists and provide oversight on the team’s most complex and high-risk cases. You will work with colleagues in the management of support cases, providing advice, coaching, resources, and other practicalsupport to minimise risks and achieve positive outcomes for customers.A key aspect of this role will be receiving referrals from across MTVH, conducting initial triage to assess case level and setting initial outcomes for the customer - this will be one in partnership with colleagues. The Assessment and Support Team will determinethe most appropriate case handler and overall approach to case management. As Team Leader, you will have oversight of Specialists who will provide coordination and support for complex cases where multiple teams/agencies are involved – e.g., through case conferencingor internal escalation routes.   • You should demonstrate expert specialist knowledge in one or more of the following areas:
  1. Domestic Abuse,
  2. Mental Health, Adult Social Care (in particular, unmet care needs),
  3. Care & Support service delivery or ASB.
  • As well as knowledge of and experience in multi-agency case management and risk assessment frameworks, such as MARAC would be helpful.
The nature of this role means that the post holder will be regularly involved in activities with vulnerable adults who receive social care. To meet our commitment to providing safe, high quality services to our customers we will complete a basicbackground check with the Disclosure and Barring Service once an offer of employment is made.
