Job Description

A new and exciting opportunity has arisen for a full time senior occupational therapist to join our Crisis Resolution and Home Treatment Team (CRHTT) Covering Caerphilly borough. The post holder will be based in Ystrad Mynach at Ysbytty ytrad Fawr. The crisis home treatment teams are currently undergoing a significant transformation, offering 24 hour crisis assessment and ensuring dedicated multi-disciplinary home treatment as an alternative to hospital admission where indicated.The purpose of the CRHTT is to provide assessment and intervention for people who are experiencing an acute mental health crisis. The CRHTT will provide information, support and treatment in the least restrictive setting and ensure that all service users are assessed for an alternative to inpatient care. The CRHTT believes that each individual has the necessary strengths to overcome such a crisis should they be given the opportunity with the right support. The team will work in collaboration with the service user, their family and others within their social and professional support network to support recovery. The CRHTT will also work alongside the Community Mental Health Teams to provide intensive relapse prevention work with the aim of preventing crisis situations and promoting recovery.The post holder will work under the supervision of the Professional Lead Occupational Therapist (OT) for the borough and will have additional opportunities to provide professional supervision for band 5 OT’s and OT support workers as required. These posts are new in the teams and a significant amount of development work in relation to the role will be supported with the other senior OTs based in Newport and Torfaen.The post holder will be required to conduct specialist OT assessments and interventions to inform the overall assessment and recovery plan for service users in collaboration with the rest of the team. This includes facilitating an early discharge home from the ward, where admission has been unavoidable, for service users who have been identified as having occupational needs. In these circumstances the post holder will work closely with the OT inpatient team and the wider multi-disciplinary team (MDT).The post holder will refer into and contribute to the running of occupational focused Recovery Through Activity (RTA) programmes which are now established in the community. The use of MOHO assessment tools and underpinning theory is currently being developed in practice across the OT service with training available.The post holder will work within an MDT of nursing, social workers, medical and psychology staff providing assessment and recovery focussed support for adults accessing the service. The teams have developed excellent links with local third sector organisations and community resource in order to promote opportunities to engage in meaningful occupation and to access the right support at the right time.The post holder will be supported by a wider network and professional structure of OTs across adult mental health in the health board area and there are regular practice forums and workshop opportunities to shape and drive OT practice.
