Clinical Nurse Lead –Brighton Band 6 £35,000-£45,000Full time, 40hours a weekOffice opens Monday-Friday and post holder is required to work 5 days working days.Better Healthcare Specialist Care Division support service users with complex Long term conditions in their own homes. Our service users are funded by either Social Services, NHS or privately funded. We work closely with the CCG and other community professionals to ensure service users complex care needs are managed at home effectively and to reduce hospital admissions.We are looking for enthusiastic, highly skilled and effective Clinical Lead to inspire and support the Community Specialist Care Team in the Brighton area. We require Clinical experience demonstrating Peg, Tracheostomy and suctioning. You would need to have ITU experience or relatable experience and be willing to travel to clients homes.You will be working closely with the Case Managers and Team leaders to ensure the delivery of high quality care for our Service users. This will include working with clinicians to gain and embed skills and competencies and also providing mentorship and support to new members of the team.The successful candidate will be a registered nurse band 5 or 6 and have an advanced understanding of the principles of effective assessment and case management and be able to demonstrate the skills and ability to clinically lead the team. You will need to be an excellent communicator, have at least 2 years in a community setting and have experience of managing a clinical caseload. We are very forward looking and innovative, therefore your enthusiasm and ideas will be an asset to the team.You would have responsibility for planning, managing and co-ordinating the care of people with long term conditions (LTC) within a defined caseload, responding to their acute needs and deterioration where appropriate. You would need to use case management techniques to reduce unplanned hospital admissions. Inherent in the role is the requirement for the post holder to develop care workers to be competent to enable them to support clients with complex care needs, care co-ordination, managing cognitive impairment, supporting self-care, management and leadership skills .The post holder should also have a proven record of strong leadership as a requirement of the role is to work effectively with other disciplines and support other leads in service provisionPlease send CV to Required skills
