Job Description

Discharge to Assess Coordinator needed with South Gloucestershire CouncilStartDate- ASAPLocation - KingswoodDuration - Initially 3 monthsHoursperweek - 37 hours per weekSalary - £16 ltd an hourDescription:

  • You will work to coordinate the Discharge to Assess cases for Pathways One and Two.
  • You will work in a proactive way to collate information from a number of relevant sources to provide the People Division with all the information that they require to efficiently facilitate discharges from Pathway One and Two and to report on the statusor all cases
  • Attending all the relevant Multi-disciplinary meetings at the relevant care homes so that accurate records are maintained
  • Keep the specialist spreadsheets updated on Pathway One and Two cases
  • To liaise with Sirona on the Delayed Transfers of Care. To check the data provided and to ensure accurate weekly reports
  • Act as a central point of contact for both Sirona and the Community Teams for information on the progress of Pathway One and Two cases
  • Monitor the progress of Pathway One and Two cases, and provide progress reports to the relevant managers
  • Regularly liaise with the brokerage team to provide information on the cases awaiting services and to ensure that they are treated as a priority
What is required
  • Educated to GCSE Level A-C in English and Maths
  • Experience of working in a similar environment or within a similar role
  • Able to collate information from a variety of sources and present findings
  • Understanding of the needs of service users and good interpersonal skills
