Job Description

We are dedicated to supporting service users with mental health and dual diagnosis. We support service users to access the community for various events such as volunteering, college, personalappointment such as GP, hospital appointments and reviews. We also support them with their everyday activities such as budgeting, their wellbeing and to live independently.A day in this role looks like….As a Support Worker at Hestia, you will play a crucial role in our Mental Health service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.A day will never look the same and some of the things you can expect to do/be responsible for are:

  • Be responsible for the support and well-being of all service users, using person-centred planning and a key-work system designed to enable them to attain and sustain a tenancy, enjoy a good quality of life and avoid hospitalisation or relapse.
  • Assess referrals, formulate and review support plans, risk management plans and move-on plans with all service users.
  • Work in partnership with outside agencies involved in the care and support of service users.
  • Ensure that service users are able to attend and participate in a range of socially inclusive activities and skills development opportunities.
  • Assess and support Service Users with a range of needs, risks and aspirations including substance misuse, families, offenders and ex-offenders, multiple needs, rough sleepers, young people including those leaving care and teenage parents, those with multipleand complex needs, single homeless people with support need, adults with HIV, people with Autism Spectrum Disorder (ASD), people at risk of domestic violence or abuse, gypsies or travellers and refugees.
  • Empower the Service Users to take responsibility for their health needs particularly in relation to preventing relapse and keeping up to date with all necessary medication.
  • Support Service Users to access appropriate treatment services within the local community such as Substance Misuse Services and accompanying them if appropriate.
  • Support Service Users to maximise their financial security and independence through addressing arrears, maintaining benefit claims and accessing employment, training and volunteering opportunities to progress into paid employment.

  • Experience of working with people with a range of different support needs, aspirations for their future and associated risks.
  • Experience of delivering high quality support services to people with a range of needs and risk.
  • Good knowledge of welfare benefits and housing options and legislation related to statutory duty.
  • Knowledge of health and safety relevant to working in Service Users homes.
  • An ability to undertake client-focused key working, support planning and risk assessments to maximise Service Users independence.
  • Good understanding of the range of issues, diagnoses and treatment relevant to the client group including substance misuse.
  • Willingness to actively engage in undertaking relevant personal development and training.
  • Ability to travel throughout the Borough of operation.
  • Previous experience of using computer based systems to accurately record work and client details.
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately.
