Job Description

Job Title: Palliative Care Registered Nurse Trust Location: IpswichPay Rates: Up to £25.07 Excluding Holiday PayShift Patterns: Earlies, Lates, Long days, Weekends and Bank Holidays To work within the multi-disciplinary team (MDT), providing physical, psycho-social and spiritual care to patients and their families admitted to the IPU, with rotation to the Community Team and occasional CCU cover. Regularly to have responsibilityfor managing the nursing care delivery on specified shifts, and to participate actively at MDT meetings and decision making.The care the hospice provide is holistic, so they care for the whole person. They endeavour to cater for the physical, emotional, spiritual, psychological and social needs of patients, families and carers with the aim to give patients the best quality oflife that they possibly can, by providing care that is appropriate to their individual needs."It is like the hospice team have wrapped their wings around us, giving us such reassurance with their physical and emotional support. What youll be responsible for
  • Assessing and monitoring patient conditions
  • Checking symptoms and vital signs and arranging diagnostic tests
  • Recognising, responding to, and managing emergency situations
  • Plan, deliver and evaluate standards of evidence-based nursing care
The role will involve, but not be limited to you performing the above.Youll need to have the following qualification/experience:
  • Palliative care qualification at diploma level or above or equivalent or experience with palliative care patients e.g. District Nurse, Hospice experience, and proven ward management skills, willing to undertake rotation to Hospice Community Team as partof IPU post.
  • Experience working in end-of-life care & syringe drivers
  • Comply with NMC Professional Registration and keep up to date with current nursing practices
  • Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
  • Undertake regular mandatory training
