We are recruiting integrated care workers to provide care and therapy support in our team in the Cambridge locality working in the community within peoples homes.Previous applicants need not apply.The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital; early assessment for care needs and avoidance of hospital admission.We will provide the necessary support to ensure your development within your role.
