Job Description

About UsThe Older Peoples Mental Health Team (OPMH) is a well-established and supportive team consisting of 9 Social Workers, 2 Assistant Care Managers and one Senior Social Work Practitioner.We work with customers with both functional and organic mental health difficulties in the community and acute inpatient wards. The team specialises in the most challenging areas within mental health therefore a good knowledge of mental health and statutory frameworks including Mental Health Act and Mental Capacity Act is desirable. Four team members are Best Interest Assessors and 4 are Approved Mental Health Professionals.About The JobSalary: Grade 9 £29,577 to £32,234 Working Pattern: Full time, 37 hoursContract Length: Permanent Location: HorshamDue to promotion, a vacancy has arisen for a Social Worker in the Older Peoples Mental Health Team in the north reporting to the team manager.The successful person will be working with colleagues in Sussex Partnership Trust including Dementia and Older Peoples Mental Health Team, Intensive support team, as well as the Assessment and Treatment mental health teams.As the team covers the North, you will need to be supportive to the local workers who are based over three sites, Crawley Mid Sussex and Horsham. The work also includes hospital discharge from both functional and the dementia care units, as well as links with the acute hospital settings. You will also need a good understanding of Section 117 funding, continuing health care and the interrelationship between adult health care funding and health.This role is an exciting challenge in the field of dementia, where we have Crawley Dementia Friendly Town with the other areas also now exploring developing a Dementia friendly strategy with the residents. We are involved in projects including a local charity that supports carers and are keen to develop further links in community.Experience and SkillsKey Skills:

  • Ability to analyse and interpret people related information and data in order to prioritise needs and formulate plans for intervention and service provision.
  • Ability to work proactively as a member of the team to make an accurate assessment of needs and negotiate and implement effective plans and outcomes.
  • Ability to work in partnership with carers, colleagues and a variety of external agencies in order to deliver the most effective service.
  • Ability to communicate clearly and effectively in both written and oral format including the ability to explain complicated information to a wide range of people and negotiate and persuade others to adopt a course of action.
  • Ability to recognise the limits of own accountability and responsibility, and to seek appropriate support, advice and supervision.
  • Sound and accurate IT and keyboard skills.
  • Qualifications and/or experience:
  • A Degree in Social Work or an equivalent qualification e.g. Certificate of Qualification in Social Work or Diploma in Social Work.
  • Appropriately registered with Social Work England and responsible for maintaining registration.
  • Good, up-to-date working knowledge of legislations relevant to the specialist area of the post e.g. Care Act, Mental Capacity Act etc.
  • Valid, full driving licence.
  • Evidence of good assessment skills, including the ability to ensure regular reviews and risk management with a variety of individuals, carers or families.
  • Experience of working in partnership with those who access or need the service and maintaining a focus on the needs and strengths of the individual, and the outcomes they want to achieve, whilst handling conflict and disagreement.
  • Experience of forming effective working relationships within a multi-agency / multidisciplinary setting.
