We are creating a Lifelong Service for anyone that has a lifelong disability - learning disability, physical or sensory disability, acquired brain injury or autism - acquired before the age of 25, providing a consistent approach where we understand and workin partnership with children, young people and adults with lifelong disabilities, autism and their families to: * Offer early intervention, * Build on an individuals strengths to develop skills and independence, * Build and promote links with the individuals community * Promote and maintain safety and good health, and * Facilitate employment.About The JobSalary: £39,493 to £42,503Working pattern: Part time, 30 hours per week (4 days)Contract type: PermanentLocation: Horsham / north of West Sussex CountyAs a Senior Social Worker within a busy community team, your role will involve working with young people and / or adults from age 16 years and upwards (depending on which team you are part of) with lifelong disabilities, including physical and learning disabilityand autism. You will be responsible for achieving positive change and improved outcomes for young people. You will have a varied, complex caseload, and provide an accountable social care service to those people who access or need the service. You will be undertaking high quality assessments and reviews including mental capacity act assessments, responding to safeguarding enquiries, and working as part of a team. You will be expected to work within organisation, policy and legislative requirements.You will have the opportunity to apply your social work skills, experience and values to the work that you do in supporting young people, their support networks and your team.Experience and Skills(These will be used as the shortlisting criteria)Qualifications and/or experience: * Degree in relevant professional or equivalent qualification e.g. Certificate of Qualification in Social Work or Diploma in Social Work. * Completion of relevant post-qualifying training. * Registration with the relevant professional registration body and responsibility for maintaining registration with that body. * Experience of effective partnership working with those who access or need the service and maintaining a focus on the needs of the individual whilst handling conflict and disagreement. * Experience of forming effective working relationships within a multi-agency / multidisciplinary setting. * Advanced theoretical, practical and procedural knowledge of social work. Knowledge of current social work research, legislation, white papers and developments in best practice.
