Job Description

Fixed term until 31/03/2021Enthusiastic, confident individual required to join the Extended Community Care Team and provide assistance with our PPE Hub.Primary role of the potholder is to provide support to the community team in relation to the distribution of PPE and assist with the delivery of a reablement approach to person centred care for the community of Helensburgh and Lomond. This position will support nursing, social work and AHP teams.To organise, order and distribute PPE to the social care sector. Work with patients within their own home as required focusing on the key priority areas of maximizing independence and supporting the individual to remain in their own home. Working to help support and facilitate prompt discharge from hospital and to actively work in a preventative way to reduce re-admission.The post holder will work alongside all health and social care agencies – community and hospital, providing intervention and support to individual clients in the Helensburgh and Lomond area.You will have previous experience working in the health or social care settings, excellent communication skills and the ability to work as part of a busy multi-disciplinary team or participate in lone working. You will be expected to travel efficiently and effectively between various work locations within Argyll & Bute to meet the operational requirements of the Service.PLEASE NOTE - You should apply for this post by completing the application process on Job Train.
