Job Description

If you enjoy supporting older adults to overcome challenges and reach their full potential, dont miss out on this good opportunity.We require Adult Social Worker to work in the Hospital Discharge Team based in Merseyside.Pay rate £25PHYour key responsibilities will include:

  • The role will involve assessing people in hospital to ensure safe and timely discharge home, or an alternative care setting.
  • To be responsible for a varying caseload including safeguarding cases.
  • To carry out the statutory duties of the Directorate in relation to adults in accordance with Directorate policies and procedures.
  • To develop skills in work within a hospital setting, including supporting discharges to the community for adults
  • Where appropriate to develop special interests according to need, demand, and service priorities.
Reed are now offering a joining bonus of up to £250 for Qualified Social Workers. Terms apply.Essential Requirements:
  • SWE Registered
  • Must have at least two years of post-qualifying experience working with adults in a statutory setting
  • Must hold UK Driving License
