We are in an exciting period of transition where we are further developing our model of care and working closely with our psychology and occupational therapy colleagues to deliver bespoke assessment leading to formulation to guide further recovery oriented care. As part of Navgio Health and Social Care our rehabilitation services have lead the way in returning service users from out of area placements; forming part of an overall model of care that focuses on service users being supported closer to home, in services tailored to meet their needs; in the least restrictive environment possible. We work creatively, innovatively and flexibly within the team- with the focus being on exceptional person centred, evidence based care- always keeping the service user as the focus.
