If you think that’s you, take a look at the job specificationMENTAL HEALTH PRACTITIONERBand 5/6 dependent on experience *Registered Mental Health Nurse/ Qualified Social Worker *Within Older People’s Mental Health Services there is a vacancy for a Mental Health Practitioner (RNMH, Social Worker or Occupational Therapist) within Older Peoples Community Mental Health and Memory Service. This would be an opportunity to work within a memory service that has twice been awarded the “excellent” status through accreditation with the Royal College of Psychiatrists.The post holder, based within CMHMS, will act as care coordinator within the CPA process for service users with complex behavioural and psychological symptoms associated with Alzheimer’s disease/mixed pathology dementia or otherwise suitable for treatment with ACI and other memory enhancement medication.The post holder will be expected to work flexibly and creatively and will provide assessment, care planning and on-going interventions to older people in conjunction with other services. The post holder will have current and relevant experience of working within a Mental Health setting for older people and will have knowledge of relapse and risk management prevention and have the ability to manage change and work within a changing environment. Senior practitioners within the service will provide supervision and role development will be actively encouraged.The post holder will be committed to the principles of person centred care and be able to teach and role model to others. They will be imaginative and broad in their outlook to care provision.
