If you think that’s you, take a look at the job specificationMENTAL HEALTH PRACTITIONER*Registered Mental Health Nurse/ Qualified Social Worker *An exciting opportunity is available for a motivated and enthusiastic Mental Health Practitioner (RNMH, Social Worker or Occupational Therapist) to join the Functional Team within Older People’s Mental Health Service. The post holder, based within CMHMS, will act as care coordinator within the CPA process for service users with complex functional mental health issues. The Functional Team work in a recovery focused way with older adults who are presenting with a functional mental illness such as, but not limited to, anxiety, depression, bipolar, psychosis or early stages of mild cognitive impairment that is impacting on psychological wellbeing. Our ideal candidate would be someone who is a team player but able to also work independently demonstrating initiative in promoting recovery and relapse prevention. The candidate will have a passion to maintain independence and support quality of life in older adults with mental health problems. The post holder will be expected to work flexibly and creatively and will provide comprehensive assessments, care planning and on-going interventions to older people in conjunction with other services. The post holder will have current and relevant experience of working within a mental health setting for older people and will have knowledge of relapse and risk management prevention and have the ability to manage/change and work within a changing environment.The post holder will be committed to the principles of person centered care and be able to teach and be a role model to others. They will be imaginative and broad in their outlook to care provision. They must be able to work upon their own initiative, whilst being committed to the principles and practice of effective team working. In return, we will support the right candidate to enhance their training and develop additional skills, providing regular supervision and personal development reviews.
