This is an exciting new role for Health Professionals looking to work with our Freshney Pelham PCN.The Health and Well Being Coach will work predominantly with the Long Term Condition Team and the GP Surgeries to support people with lower levels of patient activation. They will provide patients with the confidence to manage their own health and well being whilst increasing their ability to access and utilise community support. They may also provide access to self-management education, peer support and social prescribing.You will be working alongside the Freshney Pelham District Nursing, Care Home and Long Term Condition Teams supporting patients and encouraging them to take an active role in their health.
