We have an exciting opportunity for an enthusiastic Occupational Therapist to join the Gorleston Primary Care Home. The Primary Care Home is a multidisciplinary team including Occupational Therapists, physios, community nurses, pharmacists, as well as linking with specialist teams such as continence and neuro, early supported discharge, tissue viability and good links to Primary Care. We support safe discharges home from the acute hospitals including offering short term rehabilitation support: we also assess and provide equipment to patients experiencing acute health problems or on-going deterioration due to long term conditions and to palliative patients, enabling them to remain at home as long as possible and avoid hospital admission if possible.Gorleston has a diverse population over a relatively small geographical area. Our team is friendly and supportive of each other with high quality patient care our goal.Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques. You will have the opportunity to utilise your occupational therapy skills to the full. We normally hold clinics for those where this is appropriate for a first assessment (this is currently not happening due to Covid restrictions) and also carry out home assessments.East Coast Community Healthcare (ECCH) is a social enterprise delivering NHS contracts.
