You will be referred patients who are presenting with social rather than clinical needs or whose clinical conditionis being adversely affected by social factors.   • Taking referrals from GPs and other Primary Care professionals
  • Assessing referrals in the Practice and co-producing plans of support with them
  • Developing relationships with a wide range of agencies with whom you will develop plans of support for patients
  • Maintaining clear and accurate records of your interactions.
You must have…
  • A knowledge of benefits/welfare rights systems
  • Experience of working with people in distress
  • A working knowledge of common mental health problems
  • Excellent communication skills, verbal and written
  • The ability to form and maintain professional relationships
  • Personal resilience and a sense of humour
