You will be referred patients who are presenting with social rather than clinical needs or whose clinical conditionis being adversely affected by social factors. Taking referrals from GPs and other Primary Care professionals
Assessing referrals in the Practice and co-producing plans of support with them
Developing relationships with a wide range of agencies with whom you will develop plans of support for patients
Maintaining clear and accurate records of your interactions.

You must have…

A knowledge of benefits/welfare rights systems
Experience of working with people in distress
A working knowledge of common mental health problems
Excellent communication skills, verbal and written
The ability to form and maintain professional relationships
Personal resilience and a sense of humour
