An exciting opportunity has arisen for a band 5 Social Worker to join LOOHS (Liverpool Out Of Hospital Service) Social Work Team. This is an ideal professional development opportunity for social workers from other divisions or areas who would like to undertake statutory social work or develop the necessary skill in this area of social work practice.LOOHS arepart of Community Division ICRAS (Integrated Community Reablement & Assessment Service) pathway for adults presenting to health services as experiencing an acute physical health crisis.LOOHS Social Work Service acts as a link across community and acute settings, providing short term interventions to ensure collaborative working between sectors to ensure coordination of care and discharge planning.Shortlisting is planned for 27th May 2021Interviews are planned for 1st June 2021
