Job Description

As part of Safety Improvement Programme the South west Academic Health Science Network (SWAHSN) are aiming to reduce deterioration-associated harm by improving the prevention, identification, escalation and response to physical deterioration, through better system co-ordination and as part of safe and reliable pathways of care in our region.The SWAHSN is seeking an associate for 6 months (2 days a month) to support this programme and its primary care related activities.   • Engage with primary care stakeholders at all levels of the system to gather information and inform the programme focusing on RESTORE2, TEPs and advanced care planning, and other areas specified.
  • Attend ManDetSIP Board meetings to input into the programme planning and delivery.
  • Be the go-to person for all primary care related aspects of the programme including planning, delivery and evaluation.
  • Expected activities
  • Support the testing of the PC RESTORE2 evaluation tool in the pilots sites in South Devon
  • Work with primary care focal points in each county to gather evidence of impact and benefit to primary care and be the go to person for RESTORE2 primary care related questions/issues
  • Support the Treatment Escalation Plan/Advanced Care Planning work in primary care
  • Supporting the continued spread of RESTORE2 across the three counties by engaging with Primary Care Networks (PCNs)
  • Provide PC input into the development of the Local Improvement Plan (LIP) including metrics and data
  • Provide PC input into network events
  • Advise SWAHSN on the Directed Enhanced Service (DES) and its impact on patient safety
  • Support the link between the primary care and medicines SIP which focuses on enhancing the communication between care homes, PC and pharmacists.
  • Advise on the primary care elements of the virtual ward and pulse oximetry work
  • The role will be largely delivered remotely and you will join the Patient Safety Team. All associates will be required to agree to our SWAHSN Consultancy Agreement with the SW AHSN.
