Job Description

Offender Health Care Practitioner (Development role)Band 5-6CHCP is offering a fantastic opportunity to the right candidate to have a full career development pathway working in a prison environment.This is a specialised area of work with a challenging environment but can be very rewarding for the right candidateThe prison services in HMP Hull and HMP Humber are currently transforming the service to develop a new clinical team that integrates Primary Care with Mental Health and Drug and Alcohol Rehabilitation services.The right candidate will have the opportunity to work in a fast developing service moving from this band 5 to 6 development role through to becoming a fully trained ACP following several years of ongoing training and development.We are looking for exceptionally motivated, loyal and enthusiastic individuals with appropriate clinical proficiency and experience to join the developing team of practitioners within City Health Care Partnership Community Interest Company (CHCP CIC) Offender Health Care Services.CHCP CIC is committed to providing enhanced quality healthcare services. The post holder will work as a part of a multi-disciplinary team to provide services to meet the Primary Care, Urgent Care, Mental Health and Drug and Alcohol Rehabilitation needs of those people residing in HMP Hull and HMP Humber (East Riding).Applicants must have a Nurse or Paramedic RegistrationFor the right candidate there will be further career opportunities in service to develop skills by applying for ongoing career developments as an Advanced Clinical Practitioner by completing the nationally recognised ACP University course subject to completing all competency modules leading up to this point, including a period of consolidation at each stage.This post starts as a development role. The initial role is at band 5 for a clinician who has suitable clinical knowledge who will then move to the Band 6 position on completion of university courses and in house competencies.The post holder will be required to undertake a wide range of clinical duties including but not limited to the following:

  • Telephone triage of patients using decision support software,
  • Develop skills under clinical supervision to see and treat patients presenting with minor illnesses or injuries,
  • Involvement in mental health assessments to support the Mental Health Team
  • Support the Drug and Rehabilitation Team (DART)The post holder will be required to complete university minor illness and injury qualifications alongside in-house competencies which once successfully completed the role progresses to a band 6 position.Once established as a Band 6 Practitioner the post holder will initiate clinical assessment including history taking, clinical examination, diagnosis, treatment and onward referral where appropriate using a range of advanced skills and knowledge for patients presenting with undifferentiated and undiagnosed conditions.The post holder will be responsible for ensuring that the highest standard of clinical care is provided for patients within HMP Hull and HMP Humber.Clinical practice shifts are 24 hours a day / 7 days a week 365 days of the year and include daytimes, evenings, nights, weekends and bank holidays. Applicants should only apply if you can commit to working a full range of shifts.There will also be a need to support other service areas by working across HMP Hull and HMP Humber to support any staffing shortfalls due to sickness, training or annual leave.
