County Durham and Darlington NHS Foundation Trust is one of the largest integrated care providers in England, serving a population of around 600,000 people. We are a high performing organisation with a track record of success.The pharmacy team have received funding from the Academic Health Sciences Network (AHSN) for a 12 month project to develop and implement a treatment pathway for the management of patients with hyperlipidaemia across County Durham. This is an exciting project that will support improved patient access to PCSK9 inhibitor drugs in line with NICE guidance, improve patient care and deliver against the aims of the NHS Long Term plan.We are inviting applications from internal candidates only to undertake a secondment working as part of the team to establish and deliver new models of care for patients requiring lipid management services. This will involve delivery of patient facing lipid review clinics based either virtually or at hospital sites within the Trust. Training will be available and will be delivered in October 2020 with clinics expected to commence in November/December 2020.This role provides an excellent opportunity for a current or aspiring senior pharmacist to be a key part of the project team and will enable the postholder to further develop their clinical consultation skills in an outpatient setting. You will work closely with the wider project team, the pharmacy team and clinical colleagues across primary and secondary care to develop innovative treatment pathways to support our vision of excellence in the provision of clinical pharmacy services.Important note: Please include in the supporting information of your application why you feel you would be suited this post in County Durham and Darlington NHS foundation Trust.
