1 x 18 hoursThe South Locality Older Adult Community Mental Health Team within Rotherham, Doncaster and South Humber NHS Foundation Trust has an exciting opportunity for a Band 6, Community Mental Health Nurse to join their Team. The team is based on the Tickhill road site in the Forest Gate building. Service hours are 8.30am to 5pm Monday to Friday. The Older Adult team works alongside our colleagues in Adult Mental Health and Community Nurse Teams.The South Locality Team provides support to older adults with complex and enduring mental health issues. The team offers a range of management strategies and interventions including various psychological therapies. Care and support is offered in line with the CPA approach and Recovery model. The role involves completing initial assessments for patients with memory problems and feeding back the outcome of formulation and diagnosis.Applicants must demonstrate a flexible approach, evolving around the needs of patients, their families and significant others. The role involves working as a team member, with the responsibility of a mixed caseload of functional and organic cases.We are looking for an enthusiastic person with excellent team working skills, which is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience.
