Admiral Nurse Band 6 - 18.75 hours fixed term until March 2022Working in partnership with Making Space, AGEUK, Alzheimer’s SocietyAdmiral Nurses are specialist dementia nurses who work with and support families and carers of people with dementia. Working collaboratively with other professionals, Admiral Nurses seek to improve the quality of life for people with dementia and their carers’. They use a range of interventions to help people live positively with the condition and develop skills to improve communication and maintain relationships.We are offering a new opportunity to develop your practice within a community setting and to build upon your experience of working in a specialist field. The successful applicant will work within our established team across Doncaster. The team currently consists of;1 Team manager/Admiral Nurse RMN1 Admiral Nurse/Learning disability nurse1 Admiral Nurse/Mental Health Nurse1 Admiral Nurse/General Nurse2 Dementia Advisors- RDASH1 Dementia Advisor- Making Space1 FTE Dementia Advisor- AGE UKThe service is currently based at Blossom Lodge, Tickhill Road Site, Balby, Doncaster.Covering the Doncaster area in 4 sectors; North, East, South and Central. We have an allocated Nurse and Dementia Advisor for each area.Referrals will be triaged and allocated according to need, to either Nurse or Advisor.The casework is managed by case type- Intense, Maintaining or Holding.Visits are mainly held in a community setting to suit the individual we are supporting, but mainly in the person’s home.The Doncaster Admiral service is a partnership working with RDASH, Making Space, AGE UK, Dementia UK and the Alzheimer’s Society.
