We provide easy-to-access, effective interventions that address a range of lifestyle issues. With our support, people make sustainable change that will help them improve their wellbeing, becomehealthier and reduce their chances of developing illnesses.Health Check Practitioners are critical to the successful delivery of Reed Wellbeings world class evidence-based health and wellbeing behavioural lifestyle interventions. Health Check Practitioners will work either in groups or alone (dependent on need anddelivery location) to deliver the Norfolk NHS Health Check Catch-Up Programme.Please note this a Self-Employed role paying £15 per hour for Delivery, you will also be provided with free training paid at £12 per hour and the Care Certificate Qualification. The actual hours of work will vary, however candidateswill be required to commit to a minimum of 22.5 hours a week.This is a field based rolecoveringSouth Norfolk and Breckland Local Authority Areas between the A11 and A146 - including Diss, Long Stratton, Harleston, Bungay, East Harling and Thetford. Candidates may be required to work outside their locality onan occasional basis and will receive additional pay to compensate for the increased travel.What the job is about:Comprising of both pre-booked and opportunistic health check delivery, Health Check Practitioners will be responsible for delivering our health check service to agreed quality standards to support commissioner objectives in placing prevention at the heart ofhealth and wellbeing delivery.Your day-to-day responsibilities will include:
