Due to our unique offering, we continue to expand the business and wish to appointa number of Assessors to join our team of health and social care experts. We require people who are qualified mental health nurses, social workers and occupational therapists with a strong background in service user needs assessment and care formulation.Nurse AssessorSalary: £35,000 - £45,000 per annumAs an Assessor you will lead on clinical assessments ensuring that we are in a position to deliver individualised support packages. You will then support the person and the teams to transition to Gray Healthcare through in reach in their existing placementand commencement of their care package. You will work closely with the business development and operational teams across an agreed geographical area.As a passionate professional you will have in-depth experience of assessing and working with people with a variety of complex needs around their mental health, learning disability, ABI, personality disorder, autism and forensic backgrounds. You will have anunderstanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community and a collaborative, solutions-focussed approach to any issues associated with this. You will use your applied knowledge of theMental Health Act, CTOs and the Mental Capacity Act, Deprivation of Liberty Safeguards and Best Interest Decision making to inform the direction of packages in your remit.An expert in assessment and management of risk you will be an innovative thinker and natural communicator with strong report writing skills and computer literacy and an ability to work under pressure.
