Therefore, if you are interested, please submit your application as early as possible.We have an exciting new opportunity for a passionate and compassionate individual to join us as a Dementia Adviser in our Derbyshire team.The Dementia Advisers role involves dedicated members of staff who provide 1-1 support over the phone or on a visit to a persons home. This support may include care navigation, information and advice for people diagnosed with dementia their carers and thosewho are worried about their memory loss, who may not have been formally diagnosed but who need support in obtaining a diagnosis. While also helping people to understand dementia, cope with day to day living and preparing for the future.You will also assist service users to access support and help from others, helping them to navigate other services, providing signposting and referrals, as well as facilitating support groups and other group activities. Working with partners and key agenciesis a crucial element of this role e.g., GPs, other services and sectors that support people with dementia.About you
