HOSPITAL DISCHARGE CARE ASSISTANT - £500 WELCOME BONUSBOURNEMOUTH AND CHRISTCHURCH£10.95 -£12.95 PER HOURExciting opportunity to join a professional leading Not for Profit Care provider.Must hold UK Drivers License with access to own vehicle

  • £500 Welcome Bonus
  • £10.95 per hour (weekdays) and £ 12.95 per hour (weekends)
  • They offer you 25 or 30, 35 or 40, or 48 hour permanent contract.
  • Travel time paid £10.95 per hour
  • Mileage paid at 40p per mile
  • RAC breakdown cover
  • Free DBS & uniform
  • Shifts from 07.00 - 22.00 (some flexibility and must be willing to do a mixture of these shifts, including alternate weekends)
HOSPITAL DISCHARGE CARE ASSISTANT REQUIRMENTS:You should have previous experience in providing personal care within the last 3 years, be flexible in the hours you can work, including working alternate weekends. You will need to be able to work independently and as part of a team. Our client is seekingfor a warm-hearted, compassionate and committed experienced Care Assistant to join their growing team.KEY DUTIES FOR HOSPITAL DISCHARGE CARE ASSISTANT:
  • Assist with reviewing and updating individual tailor re-enablement plans and risk assessments on a regular basis.
  • To support clients with daily living tasks such as domestic cleaning, shopping, and assisting with medication.
  • Assist clients with meal planning, preparation and cooking
  • To escort clients to appointments, GPs, hospital and other required journeys
  • Promote service users privacy, dignity, choice, safety, confidentiality and independence.
