An exciting opportunity has arisen for the role of a Specialist Support Worker to work with the Older Adult, In-Reach and Home Treatment Team.The In-Reach and Home Treatment Team is an older adult functional crisis team; intensively supporting service users who are at risk of a hospital admission.The team is based at Walton Hospital and the working hours are between 9am and 5pm Monday to Friday with a view to increase the working hours to 9am until 5pm at weekends in the future.Our geographical area includes Chesterfield, Bolsover and North East Derbyshire,The primary aim of the post is to work within the multi-disciplinary team, in order to create opportunities for service receivers to engage in plans of care that enable their recovery, and improve their quality of life and wellbeing.Key to the post is a commitment towards an understanding of equality of experience, opportunity and outcomes for service receivers.We are looking for a kind, committed, positive and compassionate person, who will be able to deliver care at times when service users and carers are feeling at their most vulnerable.As a Support Worker you will have demonstrable knowledge of working with clients with severe and enduring mental health problems.It is also essential that you have excellent communication skills and that you operate as a well-integrated team member.You will need to have the ability to transport yourself and service users across the wide geographical area without compromising service needs.For further information, please contact:Lynsey Cockerton, Interim Service Manager,orHelen Brown, Clinical Lead
