Derbyshire NHS foundation Trust is looking to recruit an enthusiastic, compassionate and recovery focused nurse to join a brand new in-reach and home treatment service for the older adult population with functional mental health problems in the Chesterfield, Bolsover, and North East area.The service consists of a multi-disciplinary team working with older adults experiencing acute mental health problems.The service acts as an alternative to hospital, facilitating early discharges from the acute setting and preventing hospital admissions where ever possible and appropriate.The team will work with a shared caseload; providing short term, intensive interventions.We are looking for an experienced mental health nurse to be part of the growth of this new service, someone who is focused on making a positive difference to people’s lives.Experience with older adults is preferred but not essential; overall experience of working with people with acute mental health problems either in a community setting or the acute setting is ideal.The team is looking for someone with recovery-focussed ideals, based on connectedness, hope, identity, meaning and empowerment.The team will initially work Monday-Friday, 9-5. However, this will expand and cover a seven day service, 365 days per year, so the ability to work shifts is required.There is an expectation that as a Lead Nurse, you will be able to make complex decisions, lead a shift, cover duty and formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions where appropriate.We’ve been rated ‘good’ overall by the CQC, with some outstanding services.
