Job Description

We have a great opportunity for someone to join as our new Care Co-ordinator - an integral and vital role in the business ? based in Stoke-on-Trent. This is the perfect role for an existing Care Co-ordinator, Care Supervisor or a Care Assistant looking to progress their career.Rewards and Benefits for a Care Coordinator:

  • The Co-ordinator holds one of the most important positions in the company as all your actions directly affect the happiness and wellbeing of both our service users and our staff.The purpose of the Care Co-ordinator role is to make sure service users receive excellent quality care and support to help them remain safe, comfortable and independent as much as they can in their own homes.From the service user?s perspective, nobody is more highly valued than the care coordinator. You are the person they rely on in order to ensure that the right care is delivered at the right time.From the carer?s perspective you are responsible for providing them with a run of visits that reflects the hours they want to work and the service users they enjoy working with.The main objective is to provide continuity and ensure the happiness of our service users and carers.Key responsibilities of a Care Coordinator:
  • Co-ordinating on a daily basis
  • Inputting of Service user information
  • To ensure that all service user visits are covered and to manage on a day to day basis
  • Scheduling service users visits
  • Covering care worker sickness
  • Inputting service user information onto the computer system
  • Advising service users of changes to carer or visit times
  • Cover staff shortages, emergencies as directed
  • Completing service users schedules and inputting staff rotas on the call monitoring system and sending these out on the call monitoring phones
  • Processing all new service users care packages including allocation / introduction of carers, ensuring visit times/days are in accordance with the care plan and service users preferences.
  • Liaising with office Manager re Annual Leave allocation
  • On call responsibilities
  • Interviewing when required
  • What we are looking for in a Care Coordinator:
  • QCF/NVQ L3 in Health and Social Care or working towards
  • Experience of communicating clearly and building positive working relationships with people who use social care services and our carers.
  • Experience of care services
  • Skills in care and risk assessments and writing care and support plans
  • Ability to plan and organise workloads effectively so that customers receive the services that they expect
  • Experience of working within health and social care is essential
  • Excellent IT and Computer skills ? Previous experience of creating / managing work schedules is essential
