Job Description

Service/Ward/Department Summary:The Home Treatment Team is an integral part of the Adult Acute and Community services within Pennine Care NHS Foundation Trust. The team will offer intensive community based support to those service users who are experiencing an acute episode of mental ill health as an alternative to admission to an inpatient bed. The team will also provide intensive support to service users who are leaving a hospital bed as part of an individual’s discharge pathway. Time limited intensive involvement from the team will help safely resolve their acute mental health needs. Where admission is unavoidable, the team will facilitate discharge at the earliest opportunity. This may involve planned supported leave prior to discharge.The main function of the team is to provide crisis management away from an inpatient setting. The team also offer a supported discharge pathway.Role Summary:To undertake allocated duties within the Home Treatment Team, including assuming responsibility for co-ordinating shifts on a rotational basis.To be involved in the ongoing development of the service.To facilitate the implementation of early discharge pathway for service users who have been identified as appropriate for this service under the direction of senior team members.To undertake accurate psychosocial / risk assessments and act as co-worker for service users who are supported by the Home Treatment Team who present with acute mental health problems or psychological distress. To work in close liaison with other members of the multidisciplinary team, including care coordinators, to implement treatment / management plans in order to facilitate Home Treatment.To undertake the role of the gatekeeper for all people requiring admission to inpatient services, including those requiring assessment under the MHA and to provide an alternative to hospital admission through Home Treatment interventions where appropriate.It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.Key Responsibilities of the Post:

  1. To develop individualised packages of care for service users with mental health problems, including signposting where necessary, within the stepped care model (NICE, 2003), Fair Access to Care Services and the Care Programme Approach.
  2. To provide intensive home treatment in service users home/community settings.
  3. To provide crisis management and short term psychotherapeutic interventions as appropriate
  4. To develop and provide a supported discharge/leave pathway for inpatients To identify the need for protection in line with safeguarding children and the Protection of Vulnerable Adult Policy and to comply with statutory obligations.
  5. To liaise and negotiate with other professions, statutory and independent agencies to ensure the best possible service is provided to service users and carers.
  6. To evaluate systematically and record, outcomes of mental health assessments, adapting care on the basis of these outcomes.
  7. To involve family and carers in the assessment and care planning with service users consent.
  8. To provide education and appropriate self-help resources to patients and carers specific to individual needs.
