Under 65 Community WorkerA huge transformation programme in Essex has led to exciting opportunities for community mental health professionals who have an interest for Dementia Care and working as part of multi-disciplinary teams. The NHS Long Term Plan commits to grow investment in mental health services and these newly developed care pathways will support the aspirations identified in the Five Year Forward View and NHS Long Term Plan.Our Community team work with individuals with the dementia and frailty and are responsible for assisting with the delivery of patient centred care under the supervision of a senior member of the team. This will include providing practical support to service users and their carers, guidance and education and assisting in their management and care based upon the individual needs.You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required.The successful candidate will be based in Mid Essex, covering Braintree, Maldon and Chelmsford.You will be part of making change, while developing your own skills and forging a progressive career pathway.
