Job Description

Community Nurses play an important role in supporting patients to remain at home. Your role as a community nurse will help the NHS meet the needs of elderly, disabled or vulnerable patients who may not be able to easily visit the GP surgery or hospital. As an essential part of the Multidisciplinary Team, you will be required to work alongside Doctors, Therapist’s, Heath Care support Workers and Social workers in delivering a gold standard approach to care to assist in optimizing independence as well as also supporting families whose relatives are approaching end of lifeAs a Community Nurse you work independently as well as in a team.
